i asked him if he preferred SBRT or long form chemo for neoadjuvant radiation treatment post chemotherapy.

he asked if its for curative or palliative treatment, i said "cure to clean up the margins before surgery"

he responded "so if the hisopathological specific is ypT0R0N0 meaning no visible tumor left, wouldnt that mean the RT got rid of it all and the surgery was absolutely completely pointless and not that the RT was there merely to "clean up margins before surgery"

My mom has a tumor and radiation as the curative option (not surgical) is not the standard of care. he is overly opinionated for someone who i do not believe is very intelligent nor a very good doctor.

his response is almost nonsensical in the course of SBRT to get an R0 resection by cleaning up the margins in the course of borderline resectable PDAC where the tumor is abutting the portal vein and splenic vein.

I gave him complete grace. I literally said that if he has RT (he throws around the word SABR casually) and his mom has a complete pathological response from RT, then it literally means surgery was completely unnecessary (as in our friend luis' case). He doesn't seem to understand this and thinks RT is "simply to clear up the margins" for surgery. In his mind this is not an insult at all to the treating radiation oncologist.

Hes's gotten confused by the statement and gone on the offensive throwing out all the insults you can see ITT. I 100% understand his situation and empathise it, but the way he has handled it is completely out of control.
