In many cases there are only one or two countries that produce a specific isotope, because it's unique to their reactor designs. I believe France is currently the world's largest supplier, but Germany also has a monopoly on some.
But sanctions on Iran leave them unable to obtain these rare isotopes. Iranians with cancer can't get lifesaving drugs. For the last 20 years, no one told you that. You're welcome
