De-couple health insurance from healthcare.
Health insurance should just be a policy like car or home insurance. You don't 'get' anything from your policy. You don't get a PCP, you aren't entitled to any medication, you don't get anything. Zero. The only thing that policy covers is if they rack up a high enough bill, say it kicks in at 10k or something, you're covered for all health expenses up to the limit of the policy. Done. Health insurance is just insurance against a massive healthcare bill. A couple hundred bucks a month in case you get in some real trouble. Pool the risk. Most people never use it, a small number of people account for all the costs. This shit is extremely stable like other insurance premiums. No hidden costs, it's basically the same price for everyone, never goes up much.
Then when it comes to actual healthcare services give everyone a monthly allowance. Don't pay the insurers or the doctors. Pay people in an account that can only be touched for health stuff. Give everyone cash to spend if they get sick. This is way more cost effective and brings the free market back into play. People just have government savings that they can use when they get sick. 99% of cases never trigger the insurance policy. People can just spend their allotment on doctors and medication and if it ever goes tits up they have an insurance policy that kicks in automatically. Insurers can never deny you coverage, your policy just triggers the moment you go over a certain amount.
Easy, we can fire all the middle men but a small few who will just soberly collect policy money forever with no growth.

The problem is the AMA and the medical schools. They should produce at least 5x as many doctors as they do and modernize their curriculum. Doctors are almost as obnoxiously stuck in their ways as lawyers, but in the case of doctors it actually matters for many reasons, most of all that they are 10x larger share of our GDP. Doctors are also even dumber as business people than lawyers are and miss out on making a shitton more money if they did this, adding 400% more doctors to meet demand and introduce more price competition, and structured it like law firms where the 20-something and 30-something have to grind themselves bald and paunchy and the 40-somethings and older just tell the youngs what to do and get paid 5x as much.

Correct which is what most people have since most people don't ever use it.
But under my vision you would dramatically reduce the cost by firing the middle man. And then you would separate the bill for every day care so the free market would take over.
It's rent seeking middle men, consolidation, and a lack of free market factors that make the costs so high. These could all be brought into line by separating everything and introducing a race to the bottom of providers. When everything is wrapped into one wholesale and guaranteed you get insurers charging anything they want and providing as little as possible. It's a game of incentives and since ACA the profit motive has reigned supreme.

Decoupling insurance from their relationships with providers so providers can now charge whatever they want? No more negotiated rates to somewhat control price?
How much stipend you giving to people? They burn through that (even more quickly now that providers aren't limited in what they charge) and now 90% of America can't afford ANY healthcare since they have no savings and haven't hit deductible. People stop getting even preventative care.
So you wanna raise the stipend instead? Congrats you basically moving towards single payer.
