I spent a day in the ER after chemo when they drew blood and found that my glucose was at 492. ER put me on a few drugs and suspects the diabetic numbers are caused by one of the chemo medications.

They put me on Metformin and Januvia and it seems to be helping. I'm down to 200's now.

Dude yeah, be thankful if you can keep it under 250. My understanding of DKA is that it can fuck you up really fast. A healthy fasting range is 90+105 and spiking maybe up to 160s if you eat a bunch of cake and drink wine. I don't have a1c charts memorized but at 200 after 12 hours without food you are well into uncontrolled diabetes territory.

Don't fall for snake oil like glipizide. All that does is jack up your insulin levels. The real issue is not the sugar but the insulin response it provokes. It sounds like maybe your liver is cranking out a ton of sugar. Think of the sugar as a proxy for measuring the amount of insulin your pancreas is producing. THAT is what you need to control.

Diabetes can be temporary. A lot of pregnant chicks get it while they're pregnant but then it goes away. Don't know how long you'll be on these meds but you don't ever want to see 300s

I think you can probably go ~1 year at those levels before it causes genuine long term insulin resistance. I don't know. The good news though is that insulin resistance is reversible. Insulin is like any other substance in that you build up tolerance over time, and it takes more and more of it to do it's job. Reduce the insulin and your tolerance drops. Right now your body is being bombarded with heavy doses of insulin, and if that keeps up you will become insulin resistant.

I only lost about 20lb and I beatit. was never really "fat" in the sense that people didn't look at me and think "there's a fat guy." I had been athletic when I was younger though and lost a ton of muscle over the years. A lot of it was about putting dat muscle back on and not eating before bed. Not smoking weed helped a ton and I'm convinced that weed-induced inertia + munchies was a major factor in my situation. Imagine a decade of couch lock and eating chips and salsa.

For me it wasn't about how much I weighed per sec but whether I was gaining or losing it. If I only lost only a pound in any given month, my glucose levels would be down significantly that same month. But also, your body consumes sugar just maintaining muscle so adding any amount of muscle helps. Your biggest muscles are in your thighs so you get a bigger ROI by targeting those. It also matters where you are fat. Abdominal fat is worse than fat in any other region. It means you have fat around your liver and other organs, which is what gets you.

It never helps to be fat or in a state of weight gain, and exercise always helps. Just because his diabetes is being a caused by a drug doesn't mean be can't do things to bring glucose down. Anything you do that consumes glucose is helpful.

Though to be honest I was never as fucked as him. I think my highest a1c was 7.2. I only broke 300 when I was sick.

It was expensive and the sensors kept falling off, but I could watch what my organs were doing in real time. There are like 8 different types of CGM you can get now, including one that's implanted under your skin so you don't have to explain it to women.

No. Just the opposite actually. Although taking injectable insulin while the pancreas is still producing 'normal' levels of insulin can pose dosing challenges and usually needs to be done under medical supervision.

Different insurance charges different prescriptions differently. Because there is no generic rDNA/analog insulin, some insurances cover it in tiers. Others have all copays for all prescriptions.

How the fuck do you think an insulin pump works if insulin is only in pens? How exactly would it get in there?

Cartridges, which is what I've always called them and every other diabetic I know. Vials to a diabetic reminds us of the bad old days of insulin administration.

Also (no doobs) but what state do you live in? Some states, like NY, mandate coverage for diabetic products and impose price limits on insulin.

I can almost guarantee you your cost is because of your prescription coverage. I do a lot of work for an insulin pricing nonprofit, and there are definitely issues in NYS.

T1 diabetic here. Was over 500 when I was diagnosed. Running 300+ glucose for a year or more might not kill you, but might have some nasty side effects if your sugars rapidly return to normal.

You mentioned being on a corticosteroid... those are often really, really bad news. People on stuff like prednisone turn into swollen-faced irritable monsters who shovel food in their mouths.

If it continues to climb I would be concerned. Also, everyone's "normal" range is not the same. Some people just naturally run slightly higher than others due to genetics.

ty, that makes sense. There were other times in the past I'd be at like 103, then other times 90, so it has varied.

I had started having sweetened condensed milk and black tea. That plus the soda. So I hope it was just a lot of dietary sugar at that time.

Fasting blood glucose is just a snapshot of what your BG was at that exact moment in time. It's hard to draw conclusions about overall health based on it (unless it's something like 200+). Just have your A1C done next time you get blood work (suprised they don't do it automatically)

It's pretty clear you aren't actually diabetic, not in the traditional sense. You basically have a male version of gestational diabetes. I've known of people on chemo that have dealt with similar issues, managed it with oral medications, and once they were off chemo, blood sugars returned to normal levels. So in that sense, don't freak yourself out and increase your stress over the fact you think you have diabetes or are destroying your pancreas or something. In the meantime, you may want to speak with an endocrinologist, most PCP and Emergency Physicians are really, really clueless when it comes to treating diabetic symptoms or issues with the pancreas/liver. A competent endo will run some special blood tests to rule out auto-immune diabetes and help you better manage the symptoms (most likely through oral medications like metformin or glimepiride). As long as you can keep your BG under 250 (ideally 200) over the short-mid term you should not suffer any permanent side affects from the hyperglycemia.
