people wouldn’t mind the costs so much if it was efficient and predictable, but it’s not. people have to go to the ER having no idea if it’s going to bankrupt them, or go in for routine tests only to get a $5k bill 90 days later

Insurance cos are a very small portion of overall healthcare costs. They're actually limited to 20% of the total cost by federal law (ACA).

https://www.healthcare.gov/health-care-law-protections/rate-review/#:~:text=The%2080%2F20%20Rule%20generally,Medical%20Loss%20Ratio%2C%20or%20MLR.

You could cut insurance out completely and it wouldn't solve the problem

People who are outraged about $500k hospital bills would still be outraged if they were $400k instead.

" that gives them an incentive to drive up prices so they can reap a larger absolute sum"

Except they have tried to crack down on the doctors and hospital admins' billing fraud forever, and every time they do, people side with the people doing the fraud. See last year's debate about them scrutinizing bills from anesthesiologists (avg yearly income: $491k per year). The doctors and hospitals just have very good PR.

I guess UnitedHealth stock is worth $400B (even after declines) because they make so little money. And the execs are certainly not taking home millions - these are regulated middlemen with no rare skills who do nothing, so that would be irrational. Let’s go after the people who actually deliver medical care!

They have to compete against insurers that don't have those business lines. And people with those other insurers' plans have the same cost issues...the cost issue isn't unique to insurers' combined with providers like UHC.

In any event Luigi didn't go after the CEO of UHC Group--he killed the CEO of their insurance division only

Shit I forgot about how the US healthcare system draws an artificial line between prescriptions and other medical care to permit us to interact with more rent-collecting middlemen who add no value. Good point!

Now tell me about the wonders of replacing old school family practitioners with shitty urgent care centers.

Otoh yes it’s mainly the parts through the system where we allow providers to charge hundreds or thousands per patient when the exact same service/procedure/drug costs less than 1% as much in every other developed economy. That plus the AMA creating artificial doctor scarcity with its wildly outdated approach to medical education/training and its obvious mandate to maintain high compensation for its existing members.

But the insurance companies are a key driver of the cost escalation and the increase in costs driven by delaying and avoiding treatment because their entire business is built on rug pulling patients and surprising them with catastrophic bullshit charges that are never revealed to the patient until well after treatment occurs. It also encourages the average patient to think like a deadbeat/system cheater and just try to avoid paying and maintain endless stamina to argue and pushback and wait them out for a settlement offer that includes a massive discount. Ie the “smart” way to deal with them is to avoid paying and make them slash their costs on the backend and depress the reimbursement rates, which of course causes providers to raise prices. It’s all utterly retarded.

Those catastrophic bullshit charges are billed by the doctors and hospitals, who refuse to rein in costs to what the insurance companies are trying to demand. So then they turn around and bill the patients.

That’s more knowable beforehand, at least between those two parties (though it’s still completely opaque to the patient of course). It’s stuff like all the in-network/out-of-network bullshit and randomly denying coverage for procedures just to see if they can get away with it that causes the most violent rug pulls on patients. ACA is shitty overall, but it did at least curb the worst abuses that insurance companies invented to fuck patients over.

Costs for medical services are so high because insurance companies have a pile of money to pay for them. Insurasnce cos represent a huge pile of money for providers, and providers price their services accordingly.

Its the same thing with tuition. Its $70k a year because the government treasury is an insurance co for schools. The schools then get what they can.

This doesn’t seem like that big of a miss…..

“ United Health for its first quarter reported adjusted earnings per share of $7.20 versus the $7.27 expected and revenue of $109.6 billion versus the $111.6 billion expected, according to Bloomberg consensus estimates.”

It’s a huge miss because the revenue is mostly pre-contracted and they guide the analysts so estimates should be approximately right. Also when a big company that needs a good quarter misses by this much it means they pulled out all the stops in an effort to hit the number and couldn’t get there even with adjustments to accruals and other bullshit. Actual results were almost certainly worse than reported which is why people dump the stock.

Yes but that is a huge miss when most of the revenue predictably recurs and the analysts have already brought down estimates in response to news and off-the-record calls with investor relations.
