they don't really use the title "emperor", which is "imperator" and was just a military commander title. They use the term "princeps", which means "first" among the senate, or people.
You're correct that Augustus was the first to use that title to refer to himself, but in no way did Caesar have less actual power, as dictator for life, before his death. Thus the distinction is in title only, and thus irrelevant.
You might say then: well what about Sulla, couldn't you call him "emperor" as well? Two differences - he was not dictator for life, and he did not get to choose his successor, as Caesar, Augustus, and all of the "emperors" did.

That directs my mind to the most incompetent rather than the most batshit crazy Emperors (though there is some overlap).
So I would go for Valentinian III who ineffectually presided for 30 years over the final collapse of the Western Empire.

Not an emperor, but the CR is the Gracchi brothers (Tiberius or Gaius take your pick). The Gracchi brothers came to power on the back of a populist movement which arose as a consequence to the growing wealth inequality in Republican Rome. After Rome became the sole superpower in the Mediterranean following the defeat of Carthage during the Punic Wars, the elite of Rome began importing slaves on a massive scale which displaced the need for traditional Roman labor, and led to the aforementioned wealth inequities. The Gracchi brothers went against the establishment Roman political class running a populist campaign. After having some success with their populist reforms, they were both assassinated by the Roman elites. All this was the precursor of the real populist revolt that was Ceaser and the fall of the Republic.
Trump, like the Gracchi brothers, ran on a populist movement. Instead of imported slaves, today, in Post Cold War America, its open border immigration that the elites use to suppress wages. There are alot of other similarities between Trump, the Gracchi brothers and post Cold War America and Post Punic War Rome, but i dont have time to write a full dissertation.

Commodus.
Commodus once ordered all the cripples, hunchbacks, and generally undesirables in the city to be rounded up, thrown into the arena, and forced to hack one another to death with meat cleavers.
He especially adored killing animals, and killed 100 lions in one day, to the spectators’ disgust. He killed three elephants singlehanded in the arena, beheaded an ostrich and laughed at the senators attending, brandishing the head and motioning that they were next.

Cassius Dio, a first-hand witness, describes him as "not naturally wicked but, on the contrary, as guileless as any man that ever lived. His great simplicity, however, together with his cowardice, made him the slave of his companions, and it was through them that he at first, out of ignorance, missed the better life and then was led on into lustful and cruel habits, which soon became second nature."[18]
