senescent cell removal by itself extends lifespan in mice by up to 35%. there are several senolytic candidate drugs now that could possibly extend median lifespan by 1-3 decades. drugs like dasatinib are not something the average boomer is going to want to take without human trials, but it seems likely that relatively benign senolytics will be available in less than 10 years.

SENS is likely taking the wrong path. The induced pluripotent stem cell research should be taken as strong evidence that epigenetic changes are the primary cause of aging. the research being done on cyclic use of reprogramming factors to undo epigenetic changes (which isn't part of the SENS research project) is more likely to lead to large increases in maximum lifespan than anything in the SENS program.

de Grey: We need to be much more precise with definitions in order to answer your question. Epigenetic changes can be classified into two main classes: shift and noise. Shift means changes that occur in a coordinated manner among all cells of a given type and tissue, whereas noise means changes that occur in some such cells but not others, increasing the variability of that type of cell. Shifts are caused by some sort of program (genetic changes to the cell's environment), so yes, they can potentially be reversed by restoring the environment and putting the program into reverse. Noise, on the other hand, is not reversible. And we have for several years worked on determining whether it happens enough to matter in a currently normal lifetime. We have not got to a definitive answer, but it's looking though no, epigenetic noise accumulates too slowly to matter, other than maybe for cancer (which, of course, we are addressing in other ways).

Feinerman: Should we use reprogramming factors to reverse the epigenetic program?

de Grey: Probably not. There may be some benefits in doing so, as a way to restore the numbers of certain types of stem cells, but we can always do that by other methods (especially by direct stem cell transplantation), so I don't think we will ever actually need to dedifferentiate cells in vivo.

I agree with him that stochastic epigenetic drift is probably not very important (like stochastic nuclear DNA damage doesn't matter in a normal human lifespan). There are, however, highly regular epigenetic changes that are strongly predictive of mortality. Old cells that are reprogrammed show almost complete reversal of the aged phenotype. Aging is mostly programmed.

His concern with in vivo reprogramming is probably related to cancer (if cells completely lose their identity cancer formation is probable). The study done using temporary application of cellular reprogramming factors suggests this isn't necessarily a major problem.
