She’s been undergoing mfolfirinox chemo for borderline resectable pancreatic cancer.

Tumor shrunk 40% and it’s operable but would require a complex vein reconstruction now.

Surgeon referred us to a radiation oncologist for radiation therapy, but because it might take a few weeks to start we’re doing a 7th round of this chemo which is absolutely brutal and my mom is 75 years old.

Neither of us expected chemo today but after the visit with the surgeon and oncologist, we’re in a 6 hour session. I’ve been with her for all of these sessions at cedars Sinai.

Colon cancer on his liver. They never found it in his colon. It started pretty small and localized. Honestly my dad didn't even know and it only got worse during chemo. He thinks that had something to do with it but it was just the timing. Its all over now and he can barely move.

I hadn't studied cancer much before this and it's a lot more malicious than I thought. The cancer cells actually fight back against the body and create a really fucked up dynamic. They trick immune cells, kind of like Jews would.

Only about 20% of PDAC patients ever get to surgery. Even then, long term outlook is grim because this cancer immediately starts spreading microscopically through the blood vessels. Mfolfirinox is the only legitimate way to defeat it.

A family friend had inoperable metastatic inoperable pancreatic cancer. Before starting chemo went on a healthy ketogenic diet and started exercising a shitload. By the time they were about to start chemo the tumor shrunk a shit load. So they held off. He kept up with the diet and exercise. Tumor shrunk to something very very small. Docs said they can and should operate to remove. He told them to fuck off and just continues the keto and exercise. But he gets it scanned every year or 6 months or something to play it safe. It's not visible on scans anymore but apparently it's still there based on blood testing or something they can do.

A clinical trial released in June showed that PAXG which is a new chemotherapy regime actually significantly outperforms the current standard of care mFOLFIRINOX in Neoadjuvant chemotherapy for borderline resectable pancreatic cancer. Although it wasn’t offered and the chemo appears to be working for my mom, I am feeling extremely guilty internally right now for not getting her into a trial that uses PAXG chemotherapy.

It’s already operable but the surgeon wants to send it to a radiation oncologist for either long chemoradiation or SBRT to make the R0 resection significantly more likely, because an R0 resection is the only hope for a cure.

The CASSANDRA trial from mid 2025 just shows PAXG outperforms in every way, localized tumor shrinking, killing micromets etc.

the data isn't out yet but I would be shocked if PAXG wasn't significantly better in preventing reoccurrence after a clean surgery and increasing long term survival rates
