Chronic illnesses usually only allow palliative care. The best rationing is to deny more than pain management to these people.

right, that's how japan works too (except their single payer scheme still has co-insurance). if you want a fancy doctor who wants to charge above rates allowed by the national healthcare system, you can go him/her and pay out of pocket. but realistically a small portion of people will be able to afford that on a regular basis. and a small portion of doctors will be able to attract enough of that client base to sustain a private practice.

True, but in many of those cases euthanasia would actually be far more humane. People don't like the reality, but in truth we absolutely should have euthanasia legal and even have doctors push for it under certain circumstances. If you're 80 with multiple organ failure then incurring $1m/day for another 30 days on life support and in a coma until you die is not actually good for anyone.
