can't go single payer without some of these characteristics but Americans approach healthcare like they're out shopping for something and want to get whatever they want, nevermind if it's a good bang for the buck. and American providers want to provide healthcare like rent seeking apes so.

95% of health is diet and exercise.

Insurance can cover catastrophic stuff well.

Chronic illnesses usually only allow palliative care. The best rationing is to deny more than pain management to these people.

right, that's how japan works too (except their single payer scheme still has co-insurance). if you want a fancy doctor who wants to charge above rates allowed by the national healthcare system, you can go him/her and pay out of pocket. but realistically a small portion of people will be able to afford that on a regular basis. and a small portion of doctors will be able to attract enough of that client base to sustain a private practice.

Cold nigga truth: 90% of health expenditures is incurred by 10% of people

As a result, this means that we could reduce costs 90% by telling the 10% to fuck off. Or like 50% if we just didn't try very hard to treat them.

10% of people at any one specific moment in time

over our lifetimes, half of us will be in that 10% at some time, usually towards the end of our lives

True, but in many of those cases euthanasia would actually be far more humane. People don't like the reality, but in truth we absolutely should have euthanasia legal and even have doctors push for it under certain circumstances. If you're 80 with multiple organ failure then incurring $1m/day for another 30 days on life support and in a coma until you die is not actually good for anyone.

They're pretty fucking rationed. It's just health isn't necessarily a function of spend. On some level obviously the sickest and most hopeless cases need the most $$$$ and as a group deliver the least on investment.

Correct. The most important contributions to long and healthy lives comes (in order) from good food, good sanitation including clean drinking water, and exercise. Medical intervention comes WAAAAYY down the list relative to those three things.
