Radiation therapy is used in a curative manner in pancreatic cancer only to shrink the tumor to get to surgery, it is not a replacement for surgery and will almost never shrink a tumor enough to replace surgery. It almost never eradicates the tumor, it shrinks it but it will grow back unless surgically removed. Can radiation therapy make a tumor disappear? Maybe one in a thousand times. To say radiation is curative and expected to make a pancreatic cancer tumor disappear is incorrect and provides false hope. It would probably be malpractice to treat with such an intent.

Pancreatic tumors are different than other tumors, they are protected by stroma which makes them resistant to chemotherapy and radiation more than other tumors.

i asked him if he preferred SBRT or long form chemo for neoadjuvant radiation treatment post chemotherapy.

he asked if its for curative or palliative treatment, i said "cure to clean up the margins before surgery"

I gave him complete grace. I literally said that if he has RT (he throws around the word SABR casually) and his mom has a complete pathological response from RT, then it literally means surgery was completely unnecessary (as in our friend luis' case). He doesn't seem to understand this and thinks RT is "simply to clear up the margins" for surgery. In his mind this is not an insult at all to the treating radiation oncologist.
