I have spent a lot of time on support forums for schizophrenia and bipolar disorder. What you describe is very common. This sounds like bipolar disorder type 1 with psychosis.
What triggered the recurrence of symptoms? Did she stop taking medication recently? She needs a medication adjustment. If you can't convince her to see a psychiatrist, you can try to have her hospitalized.
In the US, there are three big reasons for being hospitalized for mental health issues: being a threat to yourself, being a threat to others, or being unable to take care of your basic needs (like hygiene, diet, everyday tasks, etc.). You might have to work with the police if you try to have her hospitalized.

We don’t know for sure what had triggered her symptoms but we suspect she stopped taking her medication regularly with alcohol, marijuana, and possibly weight loss pills.
We have tried with the police but she isn’t too far gone to justify involuntary mental health hold. They said she was exhibiting “bizarre behavior” but not enough to take her away. She still has her job (somehow) although we’ve received reports that she’s exhibiting weird behavior there too.
She’s reasonably intelligent so she knows how to act appropriately when necessary.

If so why do you sometimes understand it’s not real but not always?

It took me over a year to even realize I was hallucinating. That's how real they sounded. Living in near-isolation seemed to delay realizing this.
Also, I experienced olfactory hallucinations and even visual hallucinations at a few points. These happened only at my very worst points, which was always right before a long hospitalization. My longest hospitalization was for around two months.
I didn't realize I had hallucinated at all until about a year into being heavily medicated. By then, the hallucinations were gone.
I've become resilient on medication. No stressors have triggered a recurrence in nearly two years.
Best thing to do if I relapse is to take me to a hospital.
