Considering Milan, Barcelona, Madrid, Naples or any other city you guys can recommend. I spent a week in Rome and while it was great, I am not sure I want to live there.

Spent a few years living in Italy (Umbria). If you want a chill laidback country lifestyle then Umbria/Tuscany are CR. If you're looking for more beach vibes I'd go down to the Amalfi coast. If you want a "big" city then Florence > Milan > Rome > Naples. If you want a medium size city Look at Lucca, Arezzo, Genoa, Ravenna.

Yes but the difference between Milan and Madrid is still astounding in terms of basic organization, cleanliness, ease of life... Getting around. Milan looks like Switzerland compared to Rome, but Madrid looks like London compared to Milan.

Obviously that's main showcase street there but pretty much the entire city operates like that with functional American style organized streets, demarcated clean sidewalks, overall structure to shops. Nowhere in Italy, even the rich northern part, is organized that way and it becomes extremely grating for living after a while if there for more than a beach vacation.

I want to have access to all touristic centers but also to have several hobbies like trekking, photography, Italian, and a few art classes in person.
I will not be going everyday to visit every tourist spot and landmark, I will not make money from being a tourist guide.
You probably don't know enough about these cities to recommend it based on the above, right? You just think the place is aesthetically pleasing and clean and has some good spots and some top landmarks.

Ho Chi Minh, Tokyo, Munich, Dublin would be next choices for me
